1、COE4CCN:SpecialtyReferralforChildrenwithMedicalComplexityQualityMeasuresModuleContentGuidelinesThisdatacollectionabstractiontoolcollectsdatatoevaluatethecoordinationofcarewhenmedicalhomeprovidersrefertheirpediatricpatientswithcomplexneedstosubspecialists.Childrenwit
2、hcomplexneeds,bydefinition,usuallyrequirespecialtycare,oftenfromseveralproviders.Thefocusofthisabstractionisthecontinuityofinformationsharedamongthereferringandspecialtyproviders.Itincludesreferralsbymedicalhomeproviderstosubspecialists,includingmentalhealthprofessi
3、onals.Itdoesnotincludereferralstodentistry/orthodontiaorancillaryservices.Anyreferraltoasubspecialistofmentalhealthproviderthatwasmadeatleast3monthspriortothedateofabstractioncanbeevaluatedwiththeabstractiontool.Patientsareeligibleforthismoduleiftheyareatleast3month
4、soldbutlessthan18yearsoldatthetimeofreferral.Thequalitymeasuresusedintheabstractiontoolapplytochildrenwhohavecomplexneeds.ThedefinitionofcomplexityusedwiththestudythatproducedthequalitymeasuresusedhereisshowninTable1.Thistableisprovidedforinformationandguidancetothe
5、userindefiningthecasesofchildrenwithcomplexneedsforwhomspecialtyreferralsmightbeevaluatedwiththistool.(Seeseparatedocumentationforspecificationsofsampleselectiondiagnosticcodes.)ThequalitymeasurescontainedinthisabstractiontoolareshowninTable2.Referralsevaluatedinthi
6、smodulearethosemadebyamedicalhomeprovidertoaphysician(e.g.,neurology,gastroenterology,geneticist,etc.)ormentalhealthspecialist(e.g.,psychiatrist,psychologist,LSW,MFCC).Itdoesnotevaluatereferralstootherancillaryservices(e.g.,PT,communityprogram)anddoesnotevaluaterefe
7、rralsmadeforthepatient’scaregiver(e.g.,caregiverreferredforcounseling).Forpatientswhoareveryyoung,manyspecialtyreferralsmayhavealreadybeenmadebythehospitalofbirth,forexample.Inthesecases,referralsevaluatedwiththismodulearelimitedtothoseoriginatingwiththemedicalhomep
8、rovider.Thesewouldincludereferralsforabrandnewproblemorforanewaspectofanongoingproblemthatisnotcurrentlyunderspecialtycareorafterabreakins
