3、CompletetheEmployeePersonalInformationsectionontheform.§SendtheThirdPartyInputFormtothethirdpartysourceofinterest3weeksbeforetheemployee’smeetingdate.§WhenyoucompletetheYear-EndPerformanceReview,considerthethirdpartyinputalongwiththeself-review,jobdescription,document

4、ationofgoalsandobjectives,feedbackdocumentedoverthecourseoftheyear,andothersourcesofinformationandobservation.§DonotattachtheThirdPartyInputFormandavoidreferringtospecificsthatwerementionedwithin.Instead,considerthisasanadditionalsourceofinformationthatwillformyourove

5、rallevaluation.§OnlyquotedirectlyfromtheThirdPartyInputFormifthethirdpartysourcehasindicatedthatyouhavetheirpermissiontoquotedirectly,andifitwouldbringadditionalvaluetotheemployee’sdevelopmentandfutureperformance.Notethatthethirdpartysourcehaseveryrighttorefusetheincl
