3、····················································································27综述幽门螺杆菌与慢性胃炎及消化性溃疡出血的联系摘要幽门螺杆菌是临床常见病原体,其通过特殊机制可持久定植于患者胃部,引起诸多消化道疾病,甚至胃肠外疾病。其中慢性胃炎及消化性溃疡出血是发病率较高,若不及时诊治可造成严重后果的临床常见病。本文将详细探讨幽门螺杆菌在上述两种疾病发生发展中所起到的独特作用,通过对其致病机制的进一步理解,将有助于临床

7、ylori,Chronicgastritis,Pepticulcerbleeding,Colonize,Pathogenicmechanism2综述幽门螺杆菌与慢性胃炎及消化性溃疡出血的联系1.幽门螺杆菌的发现及流行幽门螺杆菌(Helicobacterpylori,H.pylori)是一种普遍常见且历史悠久的微生物,58000年前从东非开始传播[1],时至今日已经感染了超过世界50%的人口。通常人类在幼年时期即被感染,若无有效治疗,将会持续终生,可以引起诸多胃十二指肠疾病甚至胃肠外疾病[2]。H.pylor
8、i目前公认唯一明确的宿主是人类,但不少研究在哺乳动物中,如猫、鼬、貂胃部亦分离出H.pylori[3]。Abdel-Moein等[4]发现罗非鱼也可能携带一种潜在的人畜共患的H.pylori,考虑H.pylori主要是一种水传播病原体,提示其可能为人兽共患病。早在1892年,意大利学者Bizzozero就通过显微镜第一次观察到有螺旋形的细菌定居于哺乳动物的胃内,并发现这种螺旋形微生物的存在与胃上皮细胞空泡形成之间
