7、”(以下简称“统一用表”)所规定的表格格式。2、“统一用表”可使用A4纸或A3纸，表格左侧应留装订线。3、使用“统一用表”应符合下列规定：（1）填写时应使用钢笔或黑色签字笔，不得使用易退色的书写材料。（2）按照表格要求的项目内容认真填写（或打印）完整,字迹要求清楚、工整，不得涂改。（3）表格中有关人员的签字手续应完备（不得盖签字印章），需盖公章的应加盖公章，不得有漏填、漏签、漏盖现象。（4）表格填写结论应准确明晰，子项填写齐全，当有空格时应在空格处划“/”表示。4、有关证明材料使用抄件或复印件时应符合下列规定。（1）抄件或复印件应清楚
