1、M型,二维,实时三平面超声评价正常人左室收缩功能一致性研究王小娟邓爱云张钲张璐任凤霞刘天蕊(兰州大学第一医院心内科,兰州市730000)[摘要]目的:探讨M型Teichholz法,二维改良Simpson法,实时三平面超声心动图(RT3PE)评价正常人左室收缩功能(LVSF)的一致性研究,进而探讨三平面超声评价左心功能的准确性。方法:采用RT3PE测量48例正常人左室舒张末期容积(LVEDV)、收缩末期容积(LVESV)、每搏量(SV)和射血分数(EF),并分别与M型Teichholz法、二维改良
2、Simpson法所测的LVEDV、LVESV、SV、EF进行比较。结果:RT3PE与二维改良Simpson法所测的LVEDV、LVESV、SV、EF相应的测量值密切相关(r=0.914,0.892,0.815,0.996),RT3PE与M型Teichholz法所测的LVEDV、LVESV、SV、EF之间相关性较好的(r=0.896,0.814,0.796,0.917),并且上述三种方法所测LVEDV、LVESV、SV、EF之间差异无统计学意义(P>0.05)。Bland-Altman一致性分析显
3、示RT-3PE所测SV和LVEF与二维改良Simpsom法测值的一致性高于M型Teichholz法。同一观察者前后2次采用RT-3PE观察结果显示LVEDV、LVESV、SV、LVEF重复测值差异无统计学意义(P>0.05),且具有良好相关性(r=0.969、0.987、0.952,P均<0.01)。结论:RT3PE能较准确评价正常人左室收缩功能,为临床上快速、简便地评价左室功能提供了一种新方法。[关键词]超声心动图;M型;二维;三平面;左室收缩功能;一致性Comparativestudyofl
