1、探讨MRI动态增强血管成像对乳腺癌的诊断价值董栋1,2,3王新怡3(通讯作者)(1山东省医学科学院山东济南250062)(2济南大学,山东省医学科学院医学与生命科学学院山东济南250001)(3山东大学附属千佛山医院山东济南250041)【摘要】目的探讨MRI动态增强血管成像中单侧乳腺血供增加对同侧乳腺癌的诊断价值。资料与方法对169例乳腺疾病患者行乳腺动态增强磁共振检查,在最大密度投影图像上进行血管计数,当血管长度≥3cm且最大直径≥2mm时,列入筛选合格范围,得到两侧的差异血管数目,即血管得分
2、。两侧乳腺合格血管差值≥2个时认为单侧乳腺血供增加。分析乳腺单侧血供增加诊断同侧乳腺癌的灵敏度、特异度,以及与病灶大小的关联。结果以乳腺单侧血供增加作为同侧乳腺癌的诊断标准的灵敏度为65%,特异度为84%,阳性似然比(+LR)为4.06,阴性似然比(-LR)为0.42。T3肿瘤的平均血管评分显著高于T1和T2肿瘤。结论MRI动态增强血管成像可以很好的显示双侧乳腺整体血供,单侧乳腺血供增加与同侧乳腺癌相关。【关键词】乳腺癌磁共振成像最大密度投影【中图分类号】R737.9【文献标识码】A【文章编号】1672
