1、白塞直匿堂睦亟±班塞生堂僮迨塞f垫塑笙2中文摘要目的观察双心室同步起搏治疗对药物难治性充血性心力衰竭的治疗效果。方法20例充血性心力衰竭患者，男14例，女6例，年龄46—78岁(63±10)岁。均有器质性心脏病，心功能(NYHA分级)Ⅲ一IV级，其中扩张型心肌病7例，缺血型心肌病13例，所有患者12导联心电图均为窦性心律，QRs时限≥120ms，左室射血分数(LVEF)≤35％，左室舒张末期内径(LVEDD)≥55mm，伴二尖瓣返流。经多种抗心衰药物治疗无效行双心室同步起搏治疗，其中1例安装三腔ICD。观

2、察患者起搏前后NYHA心功能分级、左室射血分数、6min步行试验、QRS时限、心电轴、左房内径、左室内径、左室质量指数、二尖瓣返流、心胸比例的变化。随访时间为3—50(24±12)个月。结果(1)手术结果：20例患者均顺利完成手术，无并发症，右心房、右心室和左室电极导线感知和起搏参数均符合起搏要求。(2)心功能：术后心功能平均改善I一Ⅱ级，射血分数提高(P<0．01)，6min步行距离增加(P
